Root Cause Analysis System for Incident Investigation - Advanced
February 19-21, 2025 8:30am-4:00pm Pacific Time all days
This 3-day course explores the principles, methods and tools for digging into the root causes of a workplace injury or fatality. The course begins with a basic introduction to the root cause analysis system for incident investigation and continues with a more in-depth look that will include detailed case scenarios and interactive exercises led by a dynamic and engaging instructor. Course participants will obtain data collection and analysis skills that will equip them to conduct incident investigations and to implement appropriate corrective actions. Health and safety professionals, supervisors and workers with an interest in safety and accident investigation should attend.

Audience: Safety professionals, supervisors, and leads responsible for workplace safety and incident investigation. All are welcome to register for either the Basic or Advanced courses. There are no prerequisites for either course.

CANCELLATION POLICY: In the event you cannot attend the course, there are a few options -- you can either cancel your registration, transfer your registration to another course, or transfer your registration to another person. If you choose to cancel, we must receive your cancellation in writing with at least 1 week notice, and a refund will be issued minus a $50 cancellation fee. If you do not cancel and are a no-show for the course, your tuition fee is forfeited. If you choose to transfer your registration, please state the course you are transferring out of, and the course you are transferring into. Transferring to another course is permitted only twice per course registration. If you are unable to attend the course after the second transfer, the above cancellation policy applies.

DRIVING DIRECTIONS:
FROM Interstate 5 Northbound/Southbound:
Exit at NE 45th Street. Turn East onto NE 45th Street toward the University of Washington campus. Proceed approximately two blocks until you reach Roosevelt Way NE. Turn right onto Roosevelt Way NE (one way heading south) and travel to the Roosevelt Building, 4225 Roosevelt Way NE. The Roosevelt Building is located on the west side of the road, immediately south of the UW Medical Center Roosevelt Clinic, and across the street from the University Inn.
FROM Interstate 90 or Highway 520 Westbound:
Take the Interstate 5 North Exit, and proceed north to the NE 45th Street exit. See directions above.
Parking:
Pay lot parking is available in the Sound parking garage located immediately south of the Roosevelt Building entrance.

There are several hotels within walking distance of the Roosevelt Building that offer discount rates to guests of the University of Washington. Please mention you are a guest at booking to see if the discount rate is available.
