Welcome to the Root Cause Analysis for Incident Investigation Course Series. This is a 1-day course providing a basic introduction to the root cause analysis system for incident investigation followed by a 2-day advanced course. 

Incidents rarely result from a single mistake or unsafe act. Effective root cause analysis helps organizations understand contributing factors, identify system weaknesses, and develop meaningful corrective actions. This interactive one-day course introduces participants to the foundational principles of root cause analysis and incident investigation. Leave with a basic understanding of the procedure for incident investigation using root cause analysis and an introductory understanding of appropriate preventive measures and corrective actions.

Build foundational investigation skills in the one-day course, then deepen your capability through practical experience in interviewing, analysis,  corrective action development and reporting in the two-day program.

Audience: All health and safety professionals and workers with an interest in safety and accident investigation. All are welcome to register for either the Basic or Advanced courses. There are no prerequisites for either course.

DRIVING DIRECTIONS:
FROM Interstate 5 Northbound/Southbound:
Exit at NE 45th Street. Turn East onto NE 45th Street toward the University of Washington campus. Proceed approximately two blocks until you reach Roosevelt Way NE. Turn right onto Roosevelt Way NE (one way heading south) and travel to the Roosevelt Building, 4225 Roosevelt Way NE. The Roosevelt Building is located on the west side of the road, immediately south of the UW Medical Center Roosevelt Clinic, and across the street from the University Inn.   

FROM Interstate 90 or Highway 520 Westbound:
Take the Interstate 5 North Exit, and proceed north to the NE 45th Street exit.  See directions above.

Parking:
Pay lot parking is available in the Sound parking garage located immediately south of the Roosevelt Building entrance.

Accommodations:
There are several hotels within walking distance of the Roosevelt Building that offer discount rates to guests of the University of Washington. Please mention you are a guest at booking to see if the discount rate is available.
