IPC Plant Template and Hazard Assessment Checklist

IPC Plan Template

The IPC Plan builds on a farm’s biosecurity plans for protecting herd health with the important addition of worker health.  A good IPC Plan includes practical steps to reduce the spread of germs between cows and people—and helps keep everyone on the farm healthy. It also prepares the farm to act quickly in case of an outbreak. This document is a guide to help develop your farm’s Infection Prevention and Control (IPC) plan and is a supplement to your farm's biosecurity program. You will want to print this form out and have it while working with the Self-Guided IPC Manual or while participating the IPC PowerPoint Training.

Hazard Assessment Checklist

A workplace infectious hazard assessment is an ongoing process that engages employees and considers factors on the farm, such as sources of infectious agents, routes of human exposure, and workplace environment (moisture, ventilation and dust). Identifying and implementing effective controls promotes health and well-being across the farm, families, and community. The Self-Guided IPC Manual and the IPC PowerPoint will guide you on how to complete the Hazard Assessment Checklist which helps to identify tasks, infection sources, infection controls, and protective measures for your farm. The checklist also provides fields to input information which is unique to your farm. You will also want to print this form out and have it while working with the Self-Guided IPC Manual or IPC PowerPoint Training.
